I accept the terms and conditions under which this application is made included in the ‘Membership Rules’ (the Rules) which I have read and I undertake to observe the ‘Code of Business Practice’ (the Code) and I accept the disputes Adjudication Terms of Reference (Terms of Reference) of the Offshore Financial Trade Association which I have read and I accept that the Rules, Code and Terms of Reference may, from time to time, be amended by the Board of the Association.
